Step 1: Assessment and Information Release:

• An assessment and information release form will be distributed to parents and teachers

Initial Score Assessment and Intake Consultation:

• Upon request by the parents, a doctor will conduct an initial score assessment and a consultation in the clinic. The assessment results will be provided to the parents with a recommendation for a referral to a specialist if required.

*If the request from the parents is for re-evaluation of a child’s ADHD status, then parents can request to go straight to Step 2 instead of Step 1. 

What is the process of getting an ADHD assessment done at HealthDeliver?

Step 1: Assessment and Information Release:

Initial Score Assessment and Intake Consultation:

• Upon request by the parents, a doctor will conduct an initial score assessment and a consultation in the clinic. The assessment results will be provided to the parents with a recommendation for a referral to a specialist if required.

*If the request from the parents is for re-evaluation of a child’s ADHD status, then parents can request to go straight to the specialist skipping the above step.

Step 2: Assessment:

• The specialist doctor will conduct an initial consultation either at the clinic or online.
• Based on the initial consultation, the doctor will schedule another appointment to perform a series of tests at the clinic, depending on the purpose of the evaluation. This testing session may take up to 3 hours to complete.
• The duration may vary for each patient. If necessary, the doctor may recommend splitting the assessment into multiple sessions due to attention challenges or other factors that could make it difficult to complete in one session.
• The assessment report will be sent to the parents within 2 weeks.
• A feedback consultation session will then be scheduled to discuss strategies for managing ADHD and explore treatment options.
